I know this is probably just a draft, but I'd suggest two changes: firstly, I don't see the need for both a 'comments' and 'add comment' tab - it should be one or the other; secondly I definitely prefer the text: Please add your signature and timestamp (fourth button from the right on the toolbar) after your comment.
